Life/Disability

  • Your benefit package may include Short-Term and Long-Term Disability, Group Life/AD&D and Voluntary Life/AD&D coverages. Please refer to the materials provided by your HR team and contact them with any questions.
  • If you decline enrollment in the Voluntary LTD or Life/AD&D coverage for yourself or dependents (where applicable), you will be required to submit a statement of health should you decide to apply for coverage at a later date.
  • As a reminder, please take a moment to consider and complete a beneficiary form to ensure that any proceeds paid out on your behalf will go to the person you intended. Please retain a copy for your personal records. If you do not have a beneficiary named, any life and disability proceeds paid on your behalf may be delayed.
