Medical Benefits
As a Tribal employee, Medical coverage is available through the Federal Employees Health Benefits Program (FEHB). If you are a newly eligible employee, you may apply for coverage within your eligibility period by downloading both the SF2809 enrollment form and the Chugach enrollment form. If you are a Fort Meade employee, please use the version labeled “Fort Meade.” These forms will need to be completed, signed, and returned to your Human Resources contact.
Please note, if you are enrolling a dependent you will need to submit dependent verification in the form of a marriage certificate, or birth or adoption certificate. This documentation must be submitted to Human Resources via email with your enrollment forms. All elections will remain in effect until the following Open Enrollment period (January) unless you experience a qualifying life event. These events include:
- Marriage or divorce
- Birth or adoption of a child
- Death of a dependent
- Job status change
Employee premium share will be deducted from your paycheck on a semi-monthly basis. You can review the 2023 HMO and FFS rates by accessing the links provided on this page.
